The following are forms necessary to establish a new credential file or to update or reactivate an existing credential file. The forms are in a "print only" portable document file format and cannot be saved for return sessions or submitted online. The forms will allow you to enter information directly on each form using your keyboard. You may also download the forms and work on them at your convenience. Once you have entered the information, print the forms, sign them if necessary, and deliver or mail them to Career Services, 2112 Cleveland Blvd, Caldwell ID 83605.
