All athletes at The College of Idaho are required to have an athletic physical examination prior to participation in intercollegiate athletics. These forms must be completed and signed by a physician, the athlete, and/or athlete’s parents (if the athlete is a minor) prior to their arrival on campus at the beginning of the academic year.  Returning athletes are required to have an athletic physical examination every other year, and provide current emergency information, medical history, and accurate medical insurance information each year. All athletes are required to sign the consent form for random drug testing each year. These forms must be returned into the Head Athletic Trainer and will be kept on file in the athletic training room.
